Generate a diagnostics report and contact support. They should be able to see if/what is causing it.

Have you updated your system following last week’s firmware update and after today’s app update?
App update from your App Store.
Then Settings - Manage - System update - Check for Updates. This firmware update tackles a volume issue, just not sure it will fix yours or not.
