I currently have one SYMFONISK lamp (IKEA) and I just purchased a playbase. I wanted to create surround sound, can I use a PLAY 1 or do I need another SYMFONISK? If so, do I need the lamp or could I get the bookshelf one?

Surround speakers have to be of the same model, so you’d need to get a second lamp in order to set them up as surrounds. Or a pair of other Sonos speakers.
